Please verify your insurance benefits before booking.
Many health insurance plans include acupuncture benefits; however, coverage varies considerably between individual policies.
Dr. Loop, OMD currently accepts select plans through Aetna and UnitedHealthcare/Optum, as well as authorized Veterans Affairs (VA), Personal Injury, and Workers’ Compensation cases.
How to Verify
If you plan to use health insurance, please complete the Insurance Verification Form before booking your appointment.
Once your information is submitted, your benefits will be verified with your insurance provider. You will be contacted with the results of your verification and can then proceed with scheduling your appointment.
Because insurance plans and benefits vary, verification helps determine:
- Whether your policy includes acupuncture benefits
- Whether care is covered in-network or out-of-network
- Annual visit limits
- Deductible requirements
- Copayments or coinsurance
- Any exclusions, limitations, or authorization requirements
Please do not schedule an insurance-based appointment until your benefits have been verified and you have received confirmation from the practice.
VA, Personal Injury & Workers’ Compensation
VA care requires appropriate authorization before treatment. Personal Injury and Workers’ Compensation cases may also require authorization or additional documentation.
Please submit the appropriate information or contact the practice before scheduling care through one of these programs.
Self-Pay Care
Insurance verification is not required for self-pay appointments. Patients without acupuncture benefits—or those who prefer not to use insurance—are welcome to schedule on a self-pay basis.
The practice accepts cash, check, and most major credit cards.
Verification of benefits is not a guarantee of payment. Final coverage is determined by your insurance provider according to the terms of your individual policy.
